Penarium tem lugar numa sinistra arena de circo, na qual os concorrentes, à semelhança dos Gladiadores de antigamente, são obrigados a entreter um público sádico. Neste frenético jogo 2D de ação em arena assumes o papel do nosso peculiar anti-herói, o Willy, que para sobreviver é obrigado a correr, saltar e evitar uma variedade de armadilhas assassinas. Se conseguires, o Willy torna-se Campeão de Penarium. Falha e ele morrerá da forma mais horrível, fazendo as delícias da multidão ululante.
Orienta o pobre Willy por esta loucura maléfica no jogo a solo ou em COOP para 2 em modo local e nos modos Compeitivos.
